DETERMINATION OF QUALIFICATIONS
The Board will require by confidential inquiry, documented evidence concerning a physician's professional reputation, moral and ethical character and in-hospital practice privileges from administrative officers of organizations and institutions to whom the applicant and their conduct of practice is known.
If a physician is involved in litigation or investigation regarding ethical or moral issues, the individual will not be scheduled for examination, and the application will be re-examined. The Credentials Committee usually will defer such a decision for one year to gain further information.
Falsification of data (including case lists) or evidence of other egregious ethical, moral or professional misbehavior* may result in deferral of a physician's application for at least three years.
The physician must then meet all requirements in effect at the end of the deferred period.

RULING NOT ADMISSIBLE
A physician not admitted to any examination may reapply by submitting a new application, paying the application fee, and meeting the requirements applicable at the time of the re-application.
RE-EXAMINATIONS
Physicians who are disqualified or fail to pass the written or ABC maintenance of certification examination may reapply to take either the written, ABC, or oral examination.
A written request for an application must be submitted upon notification of results.
A physician reapplying for the oral examination will again be asked to provide verification of responsibility for patient care and will again be notified in August of admissibility to the next examination.
In September, physicians accepted for re-examination for the oral examination must submit a listing of all patients dismissed from their care in all hospitals during the six months ending June 30 prior to the oral examination and a list of 20 patients from office practice.

The examination fee is due each time a physician receives notification of admissibility to take either the written or oral examination.
The oral re-examination will be conducted by examiners who have no knowledge of the fact s, circumstances or results of a physician's previous examination(s).
