It is the responsibility of the Diplomate to seek information concerning the current requirements for maintenance of certification as an obstetrician and gynecologist. The Board does not assume responsibility for notifying a Diplomate of changing requirements for admissibility to any future examination or impending loss of admissibility to any examination. |
Diplomates meeting the requirements, who wish to be considered for certificate renewal or voluntary recertification MUST request, in writing, the application which applies to that examination.
Diplomates wishing not to renew their certificate (if certification is due to expire) must also notify the Board of this in writing. This will enable the Board to update its records and help prevent unnecessary correspondence.
All requests for applications or notifications must be written and mailed or faxed to (214) 871-1943.
After receipt of the completed application and related information, and the assessment of credentials, the Board will notify Diplomates of their status and/or further actions needed to sit for certificate renewal or voluntary recertification.
